We are currently hiring part-time (approx. $20 per hour) Home Care Coordinator to work At Your Home Compassion Care. Our mission is to support independence, dignity, and quality of life despite age and physical limitations. We aim to help our clients to live independently, in their own homes, for as long as possible.
Responsibilities
· Manage home care services with case intake, admission, and care coordination.
· Develop, implement, and evaluate individualized care plans tailored to each client’s specific needs.
· Serve as a primary point of contact for clients, staff, and healthcare professionals to ensure clear communication and understanding of care needs.
· Conduct assessments of clients’ conditions and needs to recommend appropriate services and interventions.
· Monitor and supervise care staff to ensure compliance with agency policies and quality standards.
· Facilitate regular follow-up visits and evaluations to track client progress and satisfaction with services provided.
· Address and resolve any issues or concerns raised by clients or caregivers promptly and effectively.
Requirements
· Post-secondary education in nursing, social work, or a related field.
· Minimum 2 years of recent experience in community health or a related field.
· Knowledge of the healthcare delivery system and community resources.
· Case management experience or recent home care management experience.
· Excellent communication, interpersonal, problem-solving, and decision-making skills.
· Ability to work independently and as part of a collaborative team.
· Familiarity with health care regulations, standards, and best practices.
· Additional language skills are an asset.
· A valid driver’s licence and access to a reliable vehicle.
